If you have a St Mary's profile but have not yet changed it, please log in and make the changes there first, then return here to continue. If you have made any change to your St Mary's profile related to your name, title or contact details, it will show below when you are logged in.

If you are not registered on the St Mary's website, we would strongly recommend that you do. Click "Log in to Members' Area" above, then "sign up or reset password" to register first, then return here. If you are not registered and do not wish to register, please enter your new contact details below manually.

Please enter the details of the individual account holder. Please ensure that the new name aligns with that on your National Insurance card or your new full ‘official’ name. Please do not complete this form for anyone else, even if they are a spouse and/or you share a joint bank account. We can only take instructions on existing accounts from the person named on the account.
